How Do I Submit Claims to The Benefits Trust?
There are 5 ways to submit your benefits claims
- Use our popular Mobile Claims App for Apple Devices (Download for iOS) and Android Devices (Download for Android).
- Use the Brower Version of the Claims App.
- By Email: Scan and email your claim submissions to [email protected] (must include scan of all appropriate forms and receipts).
- By Fax at 905-264-1123 (must include all appropriate forms and copy of receipts).
- By Mail (must include all appropriate forms and copy of receipts – do NOT send original receipts as they will not be returned).
What Am I Covered For?
Refer to your employee benefits booklet, available from your employer, for details of your benefits coverage. Or contact The Benefits Trust directly with any questions about your coverage.
Medical Claims
Submit through The Benefits Trust Mobile Claim App, the Online Claim Submission Form, or submit by email, fax, or mail. For mail, fax, or email claims, complete and sign a Medical and Drug Expenses Claim Form (PDF) . Submit itemized receipts showing the patient name, the service performed or goods purchased, the date, and the amount paid. A cash register “tape” or credit card receipt is not sufficient for medical claims. Some medical expenses may require a written physician’s recommendation also. Refer to your benefits booklet, or call The Benefits Trust, if you have questions about your coverage.

Vision Care Claims
Submit through The Benefits Trust Mobile Claim App, the Online Claim Submission Form, or submit by email, fax, or mail. Dental Care Claims
Your dental office can submit electronically to The Benefits Trust using your policy number and certificate number, through the NDC Health/ACE network, carrier ID 610146, batch/version 2. You can also submit a Detailed Dental Claim Form (PDF) provided by your dental office through The Benefits Trust Claim App, Online Claim Submission Form or by mail, fax, email. We can pay the dentist directly if requested. If your dentist does not print their own forms, or submit electronically, you can give the dental office one of The Benefits Trust dental claim forms. Please ask your dental office to call us if they are having difficulty submitting your claim electronically.
Short Term Disability Claims
Complete and submit a Short Term Disability claim form (PDF) to The Benefits Trust within 90 days of the end of the waiting period. Claim forms can be submitted by mail, fax, or email. Please ask your doctor to complete their section clearly and in full. During the duration of a Short Term Disability claim, an updated report may be required. This form will be provided directly to you by The Benefits Trust.
